feat: meta skill to make skill

Context

Current meta-skill-creator is overly complex with 6 phases, multiple scripts, and detailed workflows. Need a simpler approach that works like existing commands (e.g., make-command.md) with interactive Q&A workflow.

Requirement

Simplify meta-skill-creator so users can easily create skills without complex tooling. When done, users can make skills using the meta skill directly.

Solution

Use the existing commands as the example pattern - interactive Q&A workflow without requiring external scripts.
